St. Croix Hospice is seeking a Hospice Transition Nurse to act as a resource RN for patients, families, hospitals, physicians and nursing homes.

You will educate referral sources about our programs, coordinate transitions of care and support admissions to hospice services, helping ensure smooth continuity of care. The role requires an active RN license, a valid driver’s license, and the ability to pass a DHS background check.

Qualifications

• Must have current Registered Nurse (RN) license in state(s) of practice.
• Ability to pass DHS background study.
• Valid State Driver’s License, access to an automobile in good working order, automobile insurance as required.

Responsabilités

• Performs as a case manager for patients in hospital settings and coordinates hospice admission.
• Conducts goals of care meetings with patients and families to explain hospice services.
• Educates facilities and other referral sources about Hospice programs and services.
• Maintains and documents visits and phone contacts with patients, families, and referral sources.
• Assists with transition of care from hospital to home or to ongoing hospice care, coordinating with care teams.
