Suncrest Hospice is seeking an RN to provide hospice care to patients in their homes and facilities, including admissions and symptom management. The role includes on-call duties after hours, requiring travel across the service area with reliable transportation.

Ideal candidates hold an RN license in the state, with on-call and teamwork experience, and comfort using electronic medical records. Competitive pay and benefits are offered by a CHAP-certified hospice.

Qualifications

• Current RN licensure in state with one year of experience preferred.
• Admissions and On-call experience preferred.
• Ability to work in a team and independently in a home-based hospice environment.

Responsibilities

• Provide care and services for hospice patients in homes, assisted living facilities, and hospitals.
• Complete hospice admissions and patient visits; respond to calls from patients and families.
• After-hours on-call seven days/nights on and seven nights/days off.
