The role covers numerous territories within our service area; travel is required to deliver consistent, high-quality hospice care.

Qualifications

• RN license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary care teams.

Responsibilities

• Provide care and services for a group of patients.
• Visit patients at home or in facilities.
• Serve as a contributing member of the interdisciplinary team.
• Work in a Resource role across assigned territories.
