Job summary

Suncrest Hospice is seeking a Registered Nurse to provide compassionate care to hospice patients in home, facility, or hospital settings. You will conduct admissions, patient visits, symptom management, and coordinate with the care team.

The role requires an active RN license, strong clinical and communication skills, and the ability to work independently and with a team. This position includes after-hours on-call, 7 days on / 7 days off, across the service area.

Qualifications

• Current RN licensure in good standing in Ohio
• Minimum 1 year of clinical nursing experience
• Admissions and on-call experience preferred
• Ability to work with a care team and independently
• Strong time management and communication skills
• Competence with electronic medical records

Responsibilities

• Provide care for hospice patients in home or facility settings
• Complete admissions and patient visits as required
• Respond to calls from patients and families and coordinate care
• Deliver symptom management and end-of-life comfort
• Travel across service area with valid license and insurance
