Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide patient care in homes and facilities across the service area. You will be part of the interdisciplinary team, delivering clinical visits and psychosocial support in accordance with CHAP standards.

This full-time, exempt RN role covers after-hours on-call visits, requires a valid RN license, reliable transportation, and the ability to work independently or with the team to ensure high-quality hospice care.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in the state of practice.
• Ability to work with a patient care team and in a 1-1 setting as needed.
• Strong time management to schedule visits and attend meetings.

Responsibilities

• Provide care and services for patients in homes, SNFs, and assisted living facilities.
• Collaborate with the interdisciplinary team for psych-social assessments and care planning.
• Visit patients after hours with the On Call Team.
