Job summary

Suncrest Hospice in Illinois is seeking an experienced Registered Nurse (RN) for after-hours on-call coverage across our service area. You will perform hospice admissions and patient visits, delivering symptom management and compassionate care in homes, assisted living facilities, and hospitals.

Collaborate with a multidisciplinary team, answer calls from patients and families, and advocate for high-quality hospice services.

Qualifications

• Current Registered Nurse (RN) licensure in the state(s) of practice with one year of experience under license.
• Strong communication and teamwork skills with patients, families, and care team.
• Ability to manage multiple visits, admissions, and on-call duties effectively.

Responsibilities

• Provide care and services for hospice patients including admissions and visits.
• Perform symptom management and hospice visits in patients' homes, assisted living facilities, and hospitals.
• Answer calls from patients, families, and customers; advocate for hospice services.
