Affinity Hospice Management LLC seeks a Hospice Baylor RN to respond to patient census and referrals after hours, providing urgent clinical and psychosocial interventions. You will manage pain and symptoms, collaborate with providers, and document care thoroughly to ensure positive patient outcomes.

The role requires a VA RN license, graduate of an accredited nursing program, and 2+ years of hospice or related nursing experience.

Competenze

• Graduate of an accredited school of nursing and licensed RN in Virginia.
• Current unencumbered Virginia RN license required.
• Two years hospice/homecare or related nursing experience preferred.
• Ability to comply with state and federal regulations and nursing scope of practice.
• Strong documentation methods and timely record-keeping.
• Reliable transportation with valid driver’s license and insurance.

Mansioni

• Respond to hospice census and referrals after hours; provide urgent clinical and psychosocial interventions.
• Deliver pain and symptom management and collaborate with the care team.
• Assess patients and families, document care accurately, and communicate changes to physicians.
• Coordinate with IDG, vendors, hospitals, and community staff for continuity of care.
• Complete required documentation and attend mandatory meetings.
• Maintain ongoing professional development and comply with licensing requirements.
