Heartland Hospice is seeking an experienced After Hours/On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. The role emphasizes compassionate care, coordination with the daytime team, and adherence to the Plan of Care.

Responsibilities include direct patient care, documentation in EMR, family education, and collaboration with interdisciplinary staff. A sign-on bonus of $7,500 is offered, with comprehensive benefits for eligible associates.

Qualifications

• Current RN license in the requested employment state.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as primary liaison between patients, families, physicians, and hospice team.
• Respond promptly to urgent calls, visits, and symptom management needs.
• Document clinical visits and communications clearly in EMR.
• Maintain Plan of Care and educate patients and caregivers.
