Responsibilities

• Responds to Hospice patient census and referrals during shifts; handles community inquiries.
• Provides urgent clinical and psychosocial interventions; manages pain and symptoms.
• Collaborates with physicians, IDG members and admins to ensure continuity of care.
• Documents care thoroughly and timely; maintains accurate clinical records.
• Communicates patient needs and changes in condition to the team and physicians.
• Assists with after-hours admissions and coordinates with vendors for DME and meds.
• Maintains professional growth through continuing education and trainings.
