Job summary

Affinity Hospice Management LLC is seeking a Baylor RN to respond to hospice patient census and family inquiries, and to provide urgent clinical and psychosocial interventions. The role involves pain and symptom management, collaboration with the Hospice Medical Director and IDG, and after-hours admissions as needed.

The RN must be a graduate of an accredited nursing program, hold a current Virginia RN license, and be able to travel to patient homes.

Qualifications

• Must understand hospice philosophy of care.
• Functions effectively as a team member.
• Knowledge of medications and therapies for pain and symptom management.
• Visits patients where they call home.
• Communicates with physicians and other team members, and administrative staff.
• Maintains patient confidentiality per HIPAA and policy.
• Graduate of an accredited nursing program.
• Current unrestricted Virginia RN license.
• Two years of hospice/homecare/related nursing experience preferred.
• Understands and complies with state/federal laws and nursing scope.
• Documentation timely and accurate per requirements.
• Must have a valid driver’s license and insured vehicle.
• Background checks required after offer.
• Excellent observation, communication, problem-solving, basic computer and math skills.
• Ability to lift, move, and transfer patients and equipment.

Responsibilities

• Responds to census and inquiries during shifts via phone/email.
• Performs urgent nursing and psychosocial assessments after hours.
• Provides safe, competent nursing care using the nursing process.
• Focuses on pain management and symptom control.
• Documents care and outcomes thoroughly and promptly.
• Communicates with physician and IDG about patient needs and changes.
• Collaborates with hospital, vendors, and community staff for after-hours care.
• Participates in required meetings and ongoing education.
