Job summary

Allied Services seeks a Hospice Liaison RN to develop referrals and facilitate admissions for inpatient hospice, home hospice, and community palliative referrals. You will evaluate referrals, complete necessary paperwork, and market services to referral sources while ensuring age-specific needs are met within Allied Services’ care levels.

The role requires an accredited nursing degree, an active PA RN license, and strong communication skills; travel to referrals sources may be required.

Qualifications

• Graduate of an accredited Nursing Program required.
• Possession of a current RN license required.
• Hospice experience preferred.
• Strong communication and interpersonal skills emphasized.

Responsibilities

• Evaluate referrals for admission to Allied Services’ hospice and palliative care levels.
• Develop relationships with referral sources and market services to facilities.
• Complete required admission paperwork and coordinate with teams.
• Collaborate with Medical Director and social workers on candidacy reviews.
• Provide ongoing follow-up for referred patients until admission or disposition.
