Job summary

Qualifications

• Active RN license in Massachusetts.
• Minimum 1–2 years nursing experience; hospice/palliative/home health preferred.
• CPR certification and current clinical competencies.

Responsibilities

• Conduct patient assessments and develop plans of care.
• Manage pain and symptoms with palliative care practices.

Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our team in Waltham, MA. Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
• Manage pain and symptom control using evidence-based palliative care practices
• Coordinate with interdisciplinary team members including physicians, social workers, and chaplains
• Provide education and emotional support to patients and their families throughout the end-of-life journey
• Document clinical findings, care plans, and patient progress accurately and in compliance with hospice regulations
• Respond to after-hours on-call needs and urgent patient situations as required
• Collaborate with community healthcare providers to ensure seamless continuity of care

Requirements

• Active RN license in the state of Massachusetts
• Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
• Strong knowledge of pain management, symptom control, and end-of-life care protocols
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently in a field-based, patient care setting
• Valid driver's license and reliable transportation required
• CPR certification and current clinical competencies
