Job summary

Solara Hospice LLC in Waltham, MA is seeking a compassionate Hospice On Call RN to join our team. This role provides expert clinical care and emotional support to patients and families during life’s most important moments, with after-hours calls and urgent visits.

You will coordinate with the interdisciplinary hospice team, document in the EMR, manage pain and symptoms, and travel to patient homes as needed. A MA RN license and 1–2 years hospice experience are preferred.

Qualifications

• Active RN license in Massachusetts.
• 1–2 years hospice, palliative care, or home health nursing preferred.
• Strong clinical assessment and critical thinking abilities.
• Experience with pain and symptom management in end‑of‑life care.
• Proficient in EMR documentation.
• Valid driver's license and reliable transportation for home visits.
• Excellent communication and ability to stay calm under pressure.
• Compassionate, patient‑centered care approach.

Responsibilities

• Respond to after-hours calls with clinical guidance and triage.
• Conduct urgent visits to assess and manage acute changes in condition.
• Administer and adjust medications for pain and symptom control.
• Coordinate with the hospice team to ensure continuity of care.
• Document assessments, interventions, and outcomes in the EMR.
• Provide compassionate end‑of‑life support and education for patients and families.
• Communicate updates to attending physicians and on‑call staff.
