Job summary

Qualifications

• Active RN license in Massachusetts
• 1–2 years nursing experience (hospice/palliative/home health preferred)
• CPR certification and up-to-date credentials

Responsibilities

• Conduct comprehensive patient assessments and develop individualized hospice care plans
• Provide skilled nursing care and symptom management for end-of-life patients in home or facility settings
• Educate patients and families on disease progression, comfort measures, and hospice philosophy
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides
• Administer and manage medications, including pain and palliative care protocols
• Document patient care accurately and in compliance with Medicare/Medicaid hospice regulations
• Provide on-call support and respond to urgent patient and family needs as needed

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized hospice care plans
• Provide skilled nursing care and symptom management for end-of-life patients in home or facility settings
• Educate and support patients and families on disease progression, comfort measures, and hospice philosophy
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides

Requirements:

• Active Registered Nurse (RN) license in the state of Massachusetts
• Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
• Strong knowledge of pain management, symptom control, and end-of-life care
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently and manage a patient caseload in a field-based setting
• Valid driver's license and reliable transportation for home visits
• CPR certification and up-to-date clinical credentials required
