Job summary

Solara Hospice LLC in Dedham, MA is seeking a compassionate Hospice RN to deliver expert clinical care and emotional support to patients and families during end-of-life care. You will work in a patient-centered environment, leading with heart and collaborating with a dedicated team.

Qualifications

• Active RN license in Massachusetts
• 1–2 years nursing experience (hospice/palliative/home health preferred)
• CPR certification and up-to-date credentials

Responsibilities

• Conduct comprehensive patient assessments and develop individualized hospice care plans
• Provide skilled nursing care and symptom management for end-of-life patients in home or facility settings
• Educate patients and families on disease progression, comfort measures, and hospice philosophy
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides
• Administer and manage medications, including pain and palliative care protocols
• Document patient care accurately and in compliance with Medicare/Medicaid hospice regulations
• Provide on-call support and respond to urgent patient and family needs as needed
