Job summary

Solara Hospice LLC is seeking a compassionate Registered Nurse (RN) to provide end-of-life care. The role involves direct patient care in home or facility settings and collaboration with a multidisciplinary team to develop tailored plans and ensure comfort.

The ideal candidate will have an active MA RN license, hospice experience preferred, strong assessment skills, and the ability to work independently while communicating effectively with patients and families.

Qualifications

• Active RN license in Massachusetts
• Hospice, palliative care, or home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients and care teams
• Ability to work independently and manage a patient caseload efficiently
• Proficiency with EMR documentation
• Valid driver’s license and reliable transportation for patient visits

Responsibilities

• Provide skilled nursing assessments and direct patient care to hospice patients in home or facility settings
• Develop, implement, and evaluate individualized patient care plans in coordination with the interdisciplinary team
• Manage pain and symptom control to maximize patient comfort and quality of life
• Educate patients and family members on disease progression, medications, and end-of-life care

• Provide emotional support and guidance to patients and families throughout the hospice journey
