Job summary

Solara Hospice LLC in Waltham, MA is seeking a compassionate Hospice RN to provide expert clinical care and emotional support to patients and families in a patient-centered setting.

The role requires independence in a field-based work environment, strong communication, and accurate documentation, with collaboration across an interdisciplinary team to deliver dignified end-of-life care.

Qualifications

• Active RN license in Massachusetts.
• Minimum 1–2 years nursing experience; hospice/palliative/home health preferred.
• CPR certification and current clinical competencies.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct patient assessments and develop plans of care.
• Manage pain and symptoms with palliative care practices.
• Coordinate with interdisciplinary team members (physicians, social workers, chaplains).
• Educate patients and families throughout the end-of-life journey.
• Document findings and care plans in compliance with regulations.
• Respond to after-hours on-call needs and urgent situations.
• Collaborate with community providers to ensure continuity of care.
