Job summary

Solara Hospice LLC in Norwood, MA is seeking a compassionate Hospice RN to join our caring team. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important journey, delivering comfort and dignity in home settings.

You will coordinate with physicians, social workers, chaplains and aides; develop individualized care plans; manage pain and symptoms; educate families about disease progression and end-of-life planning; and

Qualifications

• Active RN license in Massachusetts.
• 1–2 years nursing experience; hospice/palliative/home health preferred.
• Strong knowledge of pain management, symptom control, and end-of-life care.
• Excellent communication and interpersonal skills.
• Ability to work independently in field/home environment.
• Valid driver's license and reliable transportation.
• CPR certification and compliance with hospice regulations.

• Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
• Provide skilled nursing care including pain management, wound care, and symptom control

Requirements:
