Job summary

Solara Hospice LLC is seeking a compassionate Hospice Chaplain to join our care team in Waltham, MA. You will provide spiritual and emotional support to patients and families during life’s profound moments.

Qualifications

• Master's degree in Divinity, Theology, Pastoral Care, or a related field preferred.
• Board Certified Chaplain (BCC) credential or APC/NACC eligibility preferred.
• Experience in hospice, palliative care, or healthcare chaplaincy preferred.
• Ability to provide interfaith and non-denominational spiritual care across diverse populations.
• Strong interpersonal and active listening skills with a calm, compassionate demeanor.
• Valid driver's license and reliable transportation for patient visits.
• Proficiency in electronic health records (EHR) documentation.

Responsibilities

• Provide non-denominational spiritual care and emotional support to hospice patients and their families
• Conduct spiritual assessments and develop individualized care plans in collaboration with the interdisciplinary team
• Facilitate religious rituals, prayer, and end-of-life ceremonies according to patient and family wishes
• Offer bereavement support and grief counseling to families before and after the loss of a loved one
• Collaborate with social workers, nurses, and physicians to ensure holistic, patient-centered care
• Maintain accurate and timely documentation of chaplaincy visits and spiritual care plans; Coordinate with community clergy and faith leaders to support patients' spiritual needs
