Solara Hospice LLC in Braintree, MA is seeking a compassionate Hospice RN to provide expert end-of-life care and support to patients and families. You'll conduct assessments, develop care plans, and work with an interdisciplinary team in a meaningful setting.

The position requires an active MA RN license, 1–2 years of nursing experience, and proficiency with EMR documentation. A valid driver’s license and reliable transportation are essential for field-based visits.

Qualificações

• Active RN license in Massachusetts.
• 1–2 years nursing experience; hospice/palliative/home health preferred.
• Strong knowledge of pain management and end-of-life care.
• Excellent communication with patients, families and care teams.

Responsabilidades

• Perform comprehensive assessments and develop individualized plans of care.
• Conduct skilled nursing visits in home, facility and residential settings around Braintree.
• Manage pain and symptoms using palliative care methods.
• Educate families on disease progression and end-of-life care.
• Collaborate with physicians, social workers, chaplains and aides.
• Document findings and care plans per Medicare/Medicaid hospice guidelines.
• Provide on-call nursing support and respond to urgent needs.
