HopeHealth is seeking a Hospice Response Team Registered Nurse (RN) in Providence, RI for a full-time evening shift (3–11:30pm) with every-other-weekend rotation. This role includes a $3,000 sign-on bonus and comprehensive benefits.

You will assess patients, implement care plans, and educate families in a fast-paced, action-oriented team. The RN must be a graduate of an accredited nursing program, be licensed in RI and MA, with 1–3 years of clinical experience and strong assessment/communication

Qualifications

• Graduate of an accredited nursing program; RN required; BSN preferred.
• Licensed as a RN in Rhode Island and Massachusetts.
• 1–3 years of clinical experience; excellent assessment and communication skills.

Responsabilités

• Initial and ongoing assessment of patient needs across physical, functional, psychosocial, and environment.
• Implement plan of care, revise as needed, and plan for discharge/transfer.
• Educate patients/families on disease process and self-care techniques; administers orders from physician.
• Initiate preventive and rehabilitative nursing procedures.
• Coordinate services with interdisciplinary team; update Plan of Care as needed.
• Inform physician of changes in patient needs and outcomes.
• Coordinate scope and frequency of services based on acuity and needs.
• Collaborate to meet patient/family needs including psychosocial and spiritual support.
