Qualifikationen

• RN required; BSN preferred.
• Licensed as a Registered Nurse in Rhode Island and Massachusetts.
• Minimum of 1–3 years of clinical experience.
• Excellent nursing assessment, clinical and communication skills.

Aufgaben

• Represents HopeHealth in assigned settings and is an expert in hospice benefits.
• Presents hospice to eligible patients and caregivers highlighting benefits.
• Performs initial assessment of patient and family for needs and ADLs.
• Completes hospice admissions in compliance with Medicare/Medicaid and state regulations.
• Creates individualized plans of care documenting Local Coverage Determination.
• Uses interdisciplinary approach to develop plans of care and initiates referrals.
• Participates in weekly IDG meetings and documents in EMR.
• Ensures payer information is current and accurate; collects insurance details.
• Completes other duties including field evaluations/admissions and triage visits.
• Participates in weekend rotation and holiday coverage.
