Job summary

HopeHealth is seeking a Hospice Response Team Licensed Practical Nurse (LPN) in Providence, RI. The part-time role covers 32 hours weekly with 11:00am-7:30pm shifts and rotating weekends/holidays.

You will assess patients, implement care plans, educate families, and coordinate services within the hospice team. The successful candidate is an LPN licensed in RI and MA, with 1–3 years of clinical experience, strong assessment and communication skills, and the ability to work autonomously in a

Qualifications

• Graduate of an accredited nursing school; LPN required; BSN preferred.
• Licensed as a Practical Nurse in Rhode Island and Massachusetts.
• Minimum of 1-3 years of clinical experience.
• Excellent assessment, clinical and communication skills.
• Bilingual preferred but not required.

Responsibilities

• Initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s needs.
• Implements the individualized plan of care and recommends revisions as necessary.
• Educates patient/family/caregiver about disease process and self-care techniques.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Collaborates with the interdisciplinary team to revise the plan of care.
• Informs physician and others of changes in patient needs and outcomes.
• Coordinates scope and frequency of services based on acuity and needs.
• Coordinates care with family/caregiver to meet identified needs.
