Job summary

HopeHealth, Inc. is seeking a Hospice Response Team Registered Nurse (RN) to join our Providence, RI hospice program.

This per diem role requires strong clinical assessment skills and the ability to respond quickly to urgent patient needs, delivering compassionate, patient-centered care. The ideal candidate will be a licensed RN in Rhode Island (and Massachusetts preferred), with 1–3 years of clinical experience, excellent communication, and the capacity to educate patients and families.

Qualifications

• Registered Nurse licensed in RI (and MA preferred).
• Graduate of an accredited nursing program; BSN preferred.
• Minimum 1–3 years clinical experience.
• Excellent assessment, clinical and communication skills.

Responsibilities

• Initial and ongoing assessment of the patient’s needs and activities of daily living.
• Implements and revises the plan of care; plans for discharge/transfer as appropriate.
• Educates patient, family and caregiver on disease process and self‑care techniques.
• Initiates preventive and rehabilitative nursing procedures.
• Coordinates with the interdisciplinary team to update the care plan.
