HopeHealth is seeking a dedicated Admissions RN to provide hospice admissions at patients' homes and long-term care facilities in Massachusetts and Rhode Island.

In this full-time role, you will perform initial assessments, determine eligibility for hospice services, create individualized care plans, document in EMR, and coordinate with the interdisciplinary team. Evening shift 1:00pm-9:30pm with weekend rotation; RN licensure in RI and MA required.

Qualifications

• RN required; BSN preferred
• Licensed as a Registered Nurse in RI and MA
• Minimum 1–3 years of clinical experience
• Excellent nursing assessment, clinical and communication skills

Responsabilités

• Represents HopeHealth in assigned settings as hospice expert for the benefit.
• Presents hospice to eligible patients and caregivers highlighting service benefits.
• Performs initial patient and family assessment to determine needs and ADL impact.
• Completes hospice admissions in compliance with Medicare/Medicaid and state regs.
• Creates individualized plan of care and documents Local Coverage Determination.
• Uses an interdisciplinary approach to develop the plan of care and initiates referrals as needed.
• Maintains EMR documentation at point of care and ensures timely entries.
• Manages payer source information and coordinates insurance details with referrals.
• Participates in weekend rotation and holiday coverage as required.
