Job summary

HopeHealth is seeking a compassionate Hospice Admissions RN to perform skilled assessments and admissions for hospice services in patients' homes and long-term care facilities in Providence, RI. The role is full-time, 40 hours weekly, with 1:00pm–9:30pm shifts and rotating weekends every 4th weekend.

The position offers a $2,000 sign-on bonus and requires RN licensure in RI and MA, with BSN preferred. You’ll coordinate care plans, complete Medicare/Medicaid compliant admissions, and participate

Qualifications

• RN license in Rhode Island and Massachusetts.
• BSN is preferred.
• Strong nursing assessment, clinical and communication skills.

Responsibilities

• Represent HopeHealth in assigned settings and act as hospice benefit expert.
• Present hospice to eligible patients and caregivers highlighting its benefits.
• Perform initial patient and family assessments to determine needs and plan of care.
