Job summary

HopeHealth is seeking a Hospice Field RN for its affiliate HopeHealth Hospice & Palliative Care in Providence, RI. The Response Team RN responds to urgent patient needs with strong assessment skills and decisive action in an on-site, fast-paced setting.

This per diem role does not include benefits and requires Rhode Island (and Massachusetts) RN licensure, plus 1–3 years of clinical experience. You will collaborate with an interdisciplinary team to deliver patient-centered care across settings.

Qualifications

• RN required; license in Rhode Island and Massachusetts
• 1–3 years of clinical experience
• Excellent assessment and communication abilities
• Bilingual preferred but not required

Responsibilities

• Initial and ongoing assessment of patient needs and daily living activities
• Implement and revise the plan of care and coordinate discharges/transfers
• Educate patients/families on disease process and self-care
• Initiate preventive and rehabilitative nursing procedures
• Coordinate with the interdisciplinary team to tailor the care plan
