Job summary

Lower Cape Fear LifeCare is seeking a Hospice Registered Nurse in Conway, SC. You will provide compassionate home‑based hospice care, support patients and families, and collaborate with an interdisciplinary team to manage end‑of‑life care.

Expect to drive between patient visits and document care in real time. You will train caregivers, coordinate with physicians and staff, and participate in team meetings to ensure quality outcomes and patient comfort.

Qualifications

• Education: Graduate of an approved school of nursing (AD, Diploma or BSN).
• Licensure: Current SC nursing license.
• Experience: Two years, home care or hospice preferred.
• Tech: Ability to use a computer and document care.
• Interpersonal: Strong communication with patients, families and staff.
• Physical: Ability to lift up to 45 lb and assist patients.
• Mental: Assess, plan and implement patient care.
• Sensory: Must have good sight and hearing to assess patients.
• Driving: Must have reliable transportation; driving required.

Responsibilities

• See patients where they consider home (residence, assisted living, SNFs).
• Train caregivers in wound care, meds, personal care, and end‑of‑life expectations.
• Spend time in car between visits; mileage reimbursed.
• Coordinate care with physicians, hospice and other disciplines between visits.
• Patients assigned by director based on acuity and geography.
• Real‑time documentation; meds ordered online.
• Self‑scheduling with flexibility to support team needs.
• Be part of an IDT that meets bi‑weekly to discuss patient care.
