Job summary

Lower Cape Fear LifeCare in Conway, SC is seeking a Nurse Practitioner – Hospice & Palliative Care to deliver high-quality hospice services across home, facility, and inpatient settings. You will conduct Medicare-required hospice face-to-face encounters, collaborate with interdisciplinary teams, and provide after-hours coverage on a rotating on-call schedule.

This role also supports cross-coverage for the Palliative Care Nurse Practitioner, ensuring continuity of care and adherence to CMS

Qualifications

• Graduate of an accredited nursing program; MSN preferred.
• Current RN license in NC/SC and NP certification eligibility.
• Minimum five years general medical experience; two years as NP preferred.

Responsibilities

• Perform hospice face-to-face visits and document findings.
• Collaborate with hospice physicians, nurses, social workers, and others to plan and review patient care.
• Provide cross-coverage for the Palliative Care Nurse Practitioner; participate in on-call schedule.
• Ensure CMS and regulatory compliance and accurate electronic medical record documentation.
