Job summary

Lower Cape Fear LifeCare in Conway, SC is seeking a Nurse Practitioner to deliver high-quality hospice and palliative care across home, facility, and inpatient settings. You will conduct hospice face-to-face visits, collaborate with an interdisciplinary team, and support goals-of-care discussions.

Responsibilities include symptom management, order entry, care coordination, and after-hours coverage, with opportunities for professional growth and cross-coverage within the hospice continuum.

Qualifications

• Graduate of nursing program with NP certification and state licensure.
• Masters degree in nursing; hospice/palliative care certification preferred.
• Experience in leadership and teaching within clinical settings.

Responsibilities

• Provide hospice and palliative care across home, facility and inpatient settings.
• Conduct F2F visits, participate in IDT meetings, and support goals-of-care discussions.
• Develop evidence-based treatment plans and manage symptoms with the team.
• Complete timely documentation in the EMR and assist with forms.
• Offer after-hours coverage and cross-coverage for continuity of care.
