Job summary

A regional hospice care provider in Sandusky, Ohio is seeking a Licensed RN to provide personal care services and coordinate care for terminally ill patients. Responsibilities include assessing patient needs, providing skilled nursing services, and serving as a liaison to physicians. The role requires at least a nursing degree, current CPR certification, and preferably experience in hospice care. A valid driver's license is necessary, along with a commitment to high ethics and professionalism.

Qualifications

• Must be a licensed RN in the State of Ohio.
• Experience in hospice, home health, or oncology preferred.
• Certification in Hospice and Palliative Care desirable.

Responsibilities

• Provide prescribed medical treatment and personal care services.
• Coordinate implementation of the Plan of Care.
• Monitor and evaluate pain and symptoms.
