Job summary

Advocate Health's Blue Team - Hospice Southeast Suburbs in Illinois is seeking a full-time hospice case manager to coordinate patient and family care across hospice settings. Weekday hours are 8:00 AM to 4:30 PM with on-call requirements (12 hours) during one Winter and one Summer holiday.

Responsibilities include leading the interdisciplinary care group, updating the EHR, and communicating with physicians to ensure quality end-of-life care. External candidates may receive a $3,000 sign-on bonus.

Responsibilities

• Identify and coordinate patient and family care to support terminally ill patients and families in all hospice settings.
• Educate patients and families regarding disease processes, hospice philosophy, signs of decline and end of life, and update physicians as required.
• Continuously review patient caseload to ensure appropriate utilization of services and hospice level of care criteria.
• Lead the interdisciplinary care group (IDG) for each patient and family, develop and implement the plan of care, and coordinate services.
• Enter, complete and update documentation in the electronic health record per standards and policies.
• Document accurately patient experiences using assessments and narrative entries.
• Assess patient response to treatments and adjust interventions with proper follow-up and documentation; communicate changes to physicians and IDG.
• Supervise Home Health Aide and licensed practical nurse supervisory visits in accordance with regulations.
• Facilitate discharge planning and coordinate community resources to ensure continuity of care.
