Job summary

Advocate Health in Illinois is seeking an RN Case Manager for home health to assess and coordinate care for patients in the Waukegan, Zion, and Winthrop Harbor area. You will collaborate with LPNs and the interdisciplinary team to deliver skilled nursing visits in patients' homes.

The schedule is Monday through Friday, 8:00 AM to 4:30 PM, with weekend rotation and on-call requirements. A $3,000 sign-on bonus is available for external candidates; internal Advocate Health teammates are not

Qualifications

Current Illinois Registered Nurse (RN) license.
Graduate of an accredited school of nursing.
Minimum one year of medical-surgical nursing experience.
Current CPR certification.
Valid driver's license and reliable transportation.
Strong communication, critical thinking, and organizational skills.
Basic computer and documentation proficiency.

Responsibilities

Perform skilled nursing visits in patients' homes.
Develop individualized care plans.
Monitor patient progress and adjust care plans as needed.
Coordinate care with LPNs and the interdisciplinary healthcare team.
Educate patients and families regarding disease management, medications, treatments, and health maintenance.
Identify changes in patient conditions and implement appropriate interventions.
Coordinate referrals, discharge planning, and community resources.
Maintain accurate and timely documentation in accordance with regulatory and organizational requirements.
Promote patient safety, quality outcomes, and evidence-based nursing practice.
Participate in quality improvement initiatives and team collaboration activities.
