Job summary

Elevation Hospice of Colorado seeks a dedicated RN to provide skilled nursing care at patients' residences, coordinating with the interdisciplinary team and families to deliver compassionate end-of-life care.

The role requires an accredited nursing degree, active RN licensure, CPR, and 2 years of clinical RN experience. Proficiency with computers and MS Office is preferred; on-call duties apply.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer use; Microsoft Office experience preferred.

Responsibilities

• Initial and ongoing assessments of the impact of terminal illness on patient needs and ADLs.
• Implement the individualized POC and revise the plan as needed.
• Consult with and educate patient/family about disease, self-care, end-of-life care.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare progress notes showing progress toward goals.
• Coordinate all patient/family services with the IDT.
• Use case management and refer to other services as needed.
• Inform physician of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability and provide support throughout care.
• Communicate information using current processes and technology.
• Apply criteria for admission and re-certification to hospice care.
• Perform on-call responsibilities and provide on-call service.
