Job summary

Elevation Hospice of Colorado is seeking a compassionate Registered Nurse to provide skilled nursing care to patients at their homes in Denver, Lakewood, and Morrison. You will coordinate with the interdisciplinary team and support families through end-of-life care, ensuring comfort and dignity.

The role requires a valid RN license, CPR, and at least two years of RN experience in a clinical setting; experience in hospice/home health is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient using a computer; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Perform comprehensive assessments of physical, functional, psychosocial, and environmental needs.
• Implement the individualized plan of care and revise as necessary.
• Provide education to patients, families, and staff on disease processes and end-of-life care.
• Document progress notes and coordinate all patient/family services with IDT.
• Assist with admission criteria and eligibility for hospice services.
• Take on-call responsibilities and provide on-call service as assigned.
