Job summary

ExcelaCare Hospice seeks an experienced Registered Nurse to provide compassionate hospice care at patients' homes or facilities. You will coordinate with a multidisciplinary team, assess needs, implement care plans, and educate families on end-of-life options.

The role requires nursing licensure, CPR certification, and 2+ years of RN experience in clinical settings. Travel to local patients is expected; on-call rotation may be required.

Qualifications

• Graduate of an accredited school of nursing.
• Current valid State Nursing license (RN).
• Current CPR certification required.
• Two years of RN experience in clinical settings preferred hospice.
• Valid driver’s license with proof of auto liability insurance.

Responsibilities

• Administer hospice care to clients in home or facility settings.
• Coordinate care with a multidisciplinary team and families.
• Develop and monitor a written Plan of Care per physician orders.
• Document patient status accurately and timely.
• Educate patients and families on health plans and end-of-life care options.
• Participate in on-call rotation and maintain confidentiality.
