Job summary

Elevation Hospice of Colorado is seeking an On-Call Registered Nurse (RN) to join our compassionate hospice team. This role focuses on delivering skilled nursing care to patients in their homes on weekends (Friday/Saturday/Sunday, 10am-10pm), coordinating with the interdisciplinary team, patients, families, and referring agencies.

The ideal candidate holds an Associate or Bachelor's degree in Nursing, is a licensed RN in Colorado, has CPR certification, and brings at least two years of clinical

Qualifications

RN license current in Colorado
Two years’ RN experience in a clinical care setting; hospice/home health preferred
Associate or bachelor’s degree in Nursing
Current CPR certification
Proficient with computers; MS Office experience preferred
Strong interpersonal skills and ability to provide support to patients and families
Demonstrate skill in nursing practice and cope with family emotional stress

Responsibilities

Administer skilled nursing care in clients' residences
Coordinate care with the interdisciplinary team, patient/family and referring agency
Coordinate care and ensure smooth communication among team and family
Participate in care coordination and documentation as required
