Zusammenfassung

Compassus is seeking a Hospice Registered Nurse in Portland-East, OR to model our values and support patients and families at home. The RN functions as an IDT member, performing routine and emergency assessments, case management, education, and symptom control to maximize comfort.

Flexible scheduling and after-hours on-call are included. Ideal candidates hold an RN license with a nursing degree and at least 1 year of nursing experience, preferably in hospice or related settings.

Qualifikationen

• Associate or Bachelor's degree in Nursing required.
• Minimum of one year nursing practice preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.
• Current CPR certification required.

Aufgaben

• Completes routine and emergency assessments on each patient as indicated by departmental policy.
• Provides clinical and psycho-social services as indicated by plan of care through case management.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT, Medical Director, and attending Physician regarding measures to alleviate symptoms and monitors response.
• Provides physical and emotional interventive care to support the patient and family in crisis through case management with the psycho-social team.
• Prevents unwarranted hospitalizations by meeting patient and family needs in the home and being proactive and reactive during crisis situations.
• Case management of routine and emergency patient care.
• Collaborates with IDT to meet needs of patient for clinical and psycho-social interventions.
• Educates patient/caregiver regarding care, disease, dying process, symptom control, and wound care.
• Keeps physician and Medical Director informed of patient needs and condition.
• Ensures documentation at bedside.
• Submits IDT narratives, time logs, and all required documentation by 10 a.m. of the following day.
