Job summary

Compassus is seeking a Hospice Registered Nurse to provide compassionate, high-quality care to patients in the home setting as part of an interdisciplinary team (IDT). The role includes routine and emergency assessments, plan-of-care execution, and education for patients and families.

The RN will coordinate with the Medical Director, attendings, and the IDT to meet symptom control needs, support families in crisis, and document care details accurately.

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Active RN license in state(s) of employment.
• Current CPR certification.
• Hospice, oncology, home health and/or long-term care experience preferred.

Responsibilities

• Completes routine and emergency assessments on each patient as indicated by departmental policy.
• Provides clinical and psycho-social services as indicated by plan of care through case management.
• Records observations, treatments, and other pertinent information.
• Communicates with IDT, Medical Director, and attending Physician as directed regarding measures to alleviate symptoms.
• Educates patient/caregiver regarding care of patient, disease process, dying process, symptom control, and wound care.
