Job summary

Compassus is seeking a Hospice Registered Nurse to join our team and provide compassionate, patient-centered home-based care. As a clinician and IDT member, you will perform routine and emergency assessments, coordinate with Medical Director and attending physicians, and tailor plans of care to comfort and quality of life for patients and families.

You will participate in admissions, education, and crisis support on a flexible schedule including after-hours on-call.

Qualifications

• Associate or Bachelor's degree in Nursing required; RN license active.
• Minimum 1 year nursing practice; hospice/home health experience preferred.
• CPR certification required.

Responsibilities

• Complete routine and emergency patient assessments as per policy.
• Provide clinical and psycho-social services per plan of care.
• Document observations, treatments, and events.
• Collaborate with IDT to coordinate hospice care.
• Educate patients and caregivers on care, disease, and symptom control.
• Maintain up-to-date documentation and time logs.
