Job summary

Compassus is seeking a Hospice Registered Nurse to join our interdisciplinary team and provide compassionate, evidence-based home-based care. You will perform routine and urgent assessments, manage care plans, educate patients and families, and coordinate with the IDT to ensure comfort and quality through crisis situations.

• Nurse must hold an active RN license in state of employment.
• Minimum of 1 year nursing practice; hospice/home health experience preferred.
• Ability to educate patients and families about disease, care, and symptom control.

Responsibilities

• Perform routine and emergency patient assessments per policy.
• Deliver clinical and psycho-social care per plan of care and case management.
• Document observations, treatments, and plan of care changes.
• Communicate with IDT, Medical Director, and attending physicians about symptom relief.
• Coordinate transitions and ensure appropriate plan of care updates.
