Job summary

VITAS Healthcare Corporation seeks a Registered Nurse, RN, to coordinate and implement the hospice plan of care. You will work within an interdisciplinary team to assess needs, manage symptoms, educate families, and ensure high-quality patient-centered care in the hospice setting.

The role requires an active RN license and at least two years of hospice, home health, or community health nursing in the last five years, with flexibility to cover shifts as needed.

Qualifications

• Two years nursing experience in hospice, home health, or community health in last five years
• Current active RN license in the state of practice
• Experience with care planning and family education preferred

Responsibilities

• Coordinate and oversee patient care within the interdisciplinary team
• Perform initial and ongoing comprehensive assessments
• Document care events accurately and timely
• Educate patients, families, and caregivers on care plans and symptom management
• Participate in care planning meetings and quality/improvement activities
• Coordinate referrals to PT, OT, Home Health, Pharmacy and supplies
• Provide visits and shifts per program needs and area coverage
• Ensure adherence to hospice standards and program policies
