Job summary

UnitedHealth Group seeks a 7on/7off Afterhours Registered Nurse for hospice care, delivering comprehensive care to patients and families after regular hours. Primary focus includes triage over the phone and timely in-person visits as part of a 7 days on, 7 days off schedule.

Responsibilities include afterhours visits, crisis management, and assessing patient needs with professional dignity and compassion in home settings. RN licensure and a valid driver's license are required.

Qualifications

• Current unrestricted RN licensure in the state of practice required.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation required.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse.
• Respond to crisis calls from patients and families after hours with urgency.
• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral when assigned.
• Conduct afterhours visits including admission, death, recertification, and routine visits.
• Identify patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions and patient/family responses to hospice care.
• Provide care in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
