Job summary

The Care Team Hospice is seeking an On-Call Registered Nurse to support after-hours care in home and facility settings. This full-time role operates on a 7 on/7 off schedule with 4:00 PM–8:30 AM coverage and weekend shifts, primarily remote with field support as needed.

Qualified candidates hold an active RN license and have hospice experience. Strong clinical judgment, crisis intervention skills, and familiarity with the HCHB EMR are preferred.

Qualifications

• Active RN license required.
• Experience in hospice or palliative care preferred.
• Strong clinical judgment and decision-making skills.
• Experience using HCHB EMR is preferred.
• Must have active driver’s license and pass background checks.

Responsibilities

• Triage patient and caregiver calls after hours.
• Provide clinical guidance and symptom management.
• Complete urgent in-person visits as needed.
• Coordinate care with interdisciplinary team members.
• Document assessments and interventions in the HCHB EMR accurately.
