Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of a patient’s home care, performing admission visits and determining eligibility for home care services. You will collaborate with medical teams to deliver high-quality care, develop and implement individualized plans of care, and support patients and families in a long-term role with strong growth potential.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform the admission visit to the patient in the home and develop the plan of care to be followed.

About the position:
