Job summary

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of patient home care. You will perform admissions visits in patients’ homes and determine eligibility, then develop the plan of care to be followed.

The role emphasizes collaboration with other medical disciplines and ongoing professional development as we offer extensive training and a supportive environment.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care and interact with other medical disciplines.
• Perform admission visit to the home and determine eligibility for home care services.
• Develop and follow the plan of care for each patient.
