Job summary

Angels Care Hospice is seeking a Registered Nurse Case Manager to coordinate all aspects of in-home patient care. You will interact with other medical disciplines, perform admission visits, determine eligibility for home care services, and develop the plan of care to be followed.

Qualifications

• Graduate of an accredited nursing program (Diploma/Associate/Bachelor).
• Current state RN license.
• Current driver’s license.
• CPR certification and Hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care and interact with other medical disciplines as needed.
• Perform admission visit to the patient in the home and determine eligibility for home care services.
• Develop and follow the plan of care for the patient.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Pay Range:

• $74.00 – $90.00 per hour (Full-Time)
• $31.00 – $40.00 per hour, $60.00 – $110.00 per visit* (PRN)
