Job summary

Angels Care Hospice seeks a Registered Nurse Case Manager to coordinate in-home patient care and perform admission visits. The role requires evaluating eligibility for home care, and developing and following the plan of care in collaboration with the care team.

The RN must hold an active RN license, a valid driver’s license, and have 1 year of RN experience (2 preferred). The position includes Full Health, Dental & Vision benefits and opportunities for growth.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care and perform admission visits.
• Develop the plan of care and interact with other medical disciplines as needed to provide professional care.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.
