Job summary

Angels Care Hospice in Kentucky is hiring a Registered Nurse Case Manager to coordinate all aspects of patient home care. You will work with multiple medical disciplines to develop and implement the plan of care, and you will perform admission visits to determine eligibility.

Qualifications

• Graduate of an accredited nursing program (BSN/ASN/ Diploma).
• Current state RN license.
• Current state Driver’s License.
• One-year RN experience; two preferred.
• CPR and Hepatitis consent/declination.
• Reliable transportation with active auto liability insurance.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
