Job summary

Angels Care Hospice in Sedona, AZ is seeking a dedicated Registered Nurse Case Manager to coordinate all aspects of home-based patient care. You will perform admissions visits, assess eligibility for home health services, and develop the plan of care in collaboration with the medical team.

The ideal candidate is an RN with a current license, CPR certification, reliable transportation, and at least one year of home health experience.

Qualifications

• Graduate of an accredited diploma, associate, or baccalaureate nursing program.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year of RN experience (two preferred).
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Perform admission visits to patients in the home, determine eligibility for home care, and develop the plan of care with the medical team.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
